60 year old male with generalised weakness
This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those clinical problems with collective current best evidence based inputs.
C/o :
Decreased urinary output
Generalised weakness
Loss of appetite
Twiching movements since evening
HOPI :
Pt. was apparently asymptomatic 2 days back and then complained of generalised weakness and decreased urination, loss of appetite, since 2 days.
Then he was referred to our hospital and fluids were infused. Since evening weakness worsened and pt developed twiching movements of the left upper limb and was dehydrated.
K/C/O DM type 2 since 7 years
N/K/C/O HTN, asthma, TB, Epilepsy
Vitals :
Pallor is seen
No cyanosis
No Lymphadenopathy
No icterus
No edema
Moderate dehydration
Temp - afebrile
Pulse rate - 104 /min
Respiration rate - 18/min
BP - 90/60 mm/ Hg
Provisional diagnosis:
Hyperglycemic syndrome
